Pancreatic Polypeptide Information for Pathologists

Summary:

Pancreatic polypeptide (PP), discovered in 1968 (Endocrinology 1968;83:1323) is a 36 amino acid peptide (molecular weight 4,200 Da) produced and secreted by PP cells (originally termed F cells) of the pancreas. PP cells are primarily located in the Islets of Langerhans and represent about 10% of the islet cells (Virchows Arch A Pathol Anat Histol 1980;387:319). Pathophysiology PP is synthesized as the amino portion of a larger 95 amino acid precursor. The biological activity is located in the carboxyl hexapeptide and requires the C terminal amide.

Common Uses By Pathologists:

PP cells can be identified in pancreatic polypeptide cell hyperplasia (PPCH), PPomas (tumors composed exclusively or mostly of pancreatic polypeptide cells) and mixed pancreatic neuroendocrine tumor. Case reports 32 year old man with malignant pancreatic polypeptide secreting tumour of islet cells (HPB Surg 1993;6:301307). 37 year old woman with pancreatic polypeptide hyperplasia causing watery diarrhea syndrome (Can J Surg 1999;42:55). 46 year old woman with symptomatic pancreatic polypeptide-secreting tumor of the distal pancreas (Int J Gastrointest Cancer 2002;32:153).
